1. Your NAME and EMAIL Address
2. The Item Name (item name is listed above the picture)

3. Your Child's shirt size
4. Child's name or initial to be put on shirt                                            

                             

                            How to Pay?                     
Once you have sent us your order information,

       we will send you a paypal invoice or you can pay by check.  

Once that payment has been recieved we will get working on your shirt!
